Care Paths are the disease management programs specifically designed for the treatment of the most common home health diseases and diagnoses. Our clinicians (nurses, physical, occupational and speech therapists) initiate the Care Paths (protocols) upon admission and personalize the care to the patient’s needs. The Care Path consists of assessment, treatment and goals expected for the patient while on services. In addition, there are easy to follow teaching guides for the patient and/or family to follow.
